At Lafayette Eye Associates, we value your time and safety. In an effort to save you time in our office, electronic forms will be emailed to you in advance of your visit. We ask that you complete the electronic form at least one day prior to you scheduled. Please remember to bring your medical insurance card, vision insurance card (if applicable) and driver's license or ID with you for your appointment and to complete the patient information forms in advance of your scheduled appointment.

The benefits are many including the ability to confirm your demographic and appointment information and other pertinent information regarding your vision and eye health.
